Clinical Pharmacist-Full-Time

  • The clinic specialty pharmacist is responsible for the provision of safe, effective, and prompt medication therapy. Through various Assignments within the department.
  • Delivers a high level of service to each patient in every interaction, ensuring they perceive Baptist Health's pharmacy as the pharmacy of choice and ultimately impacting their level of success in their therapy.
  • Provides clinical consultation, chronic disease state management, medication reconciliation, and adherence counseling for specialty patients.
  • Facilitates the development, implementation, and maintenance of programs that promote desirable patient outcomes through safe, appropriate, cost-effective medication therapies.
  • This role will be physically positioned in a specialty medical clinic setting or centralized call center, and work in direct contact with the Pharmacy Care Coordinator. This role may include on call responsibility for afterhours urgent clinical escalations.

Requirements:

  • Graduate of an accredited College of Pharmacy with a BS in Pharmacy with a Pharm D preferred.
  • Kentucky and/or Indiana Board of Pharmacy license in good standing. Licensure in surrounding states might be necessary/preferred to fulfill job responsibilities.
  • Hospital experience and /or completion of an ASHP-accredited Pharmacy Residency preferred.
  • Current Basic Life Support (BLS) and advanced Cardiovascular life Support (ACLS) certification preferred.
